1. Miscellaneous forms along with copy of passport of informant.
2. Death certificate issued by the health department clearly mentioning the cause of Death
3. Passport (original and Photocopy both first and last page)
4. Embalming certificate
5. Non-contagious certificate
6. Non epidemic certificate
7. Closing of coffin certificate
8. Request letter/Authorization from the family member
9. Letter from Air cargo office for receiving Mortal remains
10. Air tickets
11. Company letter of no-dues & no pending liabilities certificate(if any)

FEE: US $ 22.00

Bank charges - US$5.00 with all the services if the fee is paid through bank draft/banker’s cheque in favour of Embassy of India, Paramaribo.
